Medical and health institutions within elderly care institutions will be incorporated into the management of medical consortia, and a two-way referral mechanism will be established with the leading hospitals, rehabilitation hospitals and nursing homes (centres and stations) within the medical consortia to provide integrated and continuous services, so as to achieve efficient synergy of medical, rehabilitation, nursing and elderly care service resources.
